The advanced clinical rotation at the Life Care Hospital unveiled a whole new world of patient care to me. I had the privilege to write orders for nutrition support for high risk patients. During this rotation I had the opportunity to interact with the multidisciplinary team and develop nutrition care plans based on the recommendations by other team members. Team work, applying MNT to complicated cases, using clinical judgment and finding the right balance when dealing with patient’s nutritional care, were the highlights of my rotation at Life Care Hospital. A case study presentation on a patient post Roux-en-Y reversal helped me to look at the whole picture while performing patient care. Interaction with the patient and the family and a thorough overview of patient’s medical history enabled me to better understand and address patient’s nutritional needs. Following is a brief overview of my experiences: · Assessed, diagnosed and implemented nutrition care plans, and monitored progress of high risk patients · Conducted initial assessments, follow ups, and nutritional education for patients · Documented diet and nutrition orders for PO diets, tube feedings and TPNs in the medical charts · Participated and assisted the physician with PEG placement procedures · Observed and assisted the Speech Language Therapist with swallow studies also observed and assisted the wound care team with wound vac procedures · Learned to check residuals with a nurse · Learned about medications used at LifeCare Hospital and designed a drug and functions refrence list · Performed and documented calorie counts · Designed a personalized menu selection chart based on exchanges for a diabetic patient · Conducted a tube feed audit for all the TF patients · Attended multidisciplinary team meetings
